Application Guidelines, Terms & Conditions – March 2023

Program Priorities

The Henry Kapono Foundation (HKF) awards up to $1,000 Grants toward the purchase of Medical Equipment for Music Industry Professionals in the state of Hawaiߵi who are currently in financial hardship. The priorities are as noted:

  • Applicants must be a full-time Music Industry Professionals of the State of Hawai`i for a minimum of 2 years

  • Applicants must be a resident of the State of Hawai`i

  • The Foundation does not accept applications from residents or Music Industry Professionals outside the State of Hawai`i

  • Applications must be complete and in compliance with the terms and conditions of the program as noted

  • HKF does not accept incomplete applications 

  • When considering requests for the award, preference is based on financial circumstances & the narrative describing the need for the EME

  • HKF will award no more than one EME grant to an individual within a 12-month period

  • Priority will be awarded to applicants who have not yet received an EME grant award that meets the established criteria

  • An Applicant who has received an award may reapply for an additional EME grant after a 12-month grace period following the receipt of an award

  • Program applications will be reviewed following the deadlines as noted in the guidelines, for consideration

  • The term of the EME Grant will be April 1, 2023, through December 31, 2023

Application Procedures for Online Submission:

  • All applications must be submitted online

  • The application has five sections:

    • Applicant Profile: About You

    • Medical Equipment Need

    • About your Household

    • Medical Insurance Survey

    • Application Certification

  • Application deadlines are on the last day of the quarter as noted in each deadline

  • Applications submitted after the last day of each deadline will automatically move into the next submission period

  • Only completed applications will be accepted

  • The online application has fillable boxes with character limits. The character counts in MS Word may not match the character counts in the application. If you cut and paste your work into the application, please be sure your text fits the space provided

  • To maximize character potential, please avoid the use of diacritical marks on the online application

  • Approved applications will be kept active within the quarter it is submitted only

  • Applicants can apply each calendar quarter if no award is received

Applicant Profile Information

As part of the online application, your profile must be personally certified prior to the submission of grant applications. Information needed to complete your profile includes, but is not limited to the following:

  • Personal contact information

  • Medical Equipment purchase information and cost

  • Estimated annual household income prior to this situation

  • Estimated household income in this situation

  • Number of members in your household: Adults & Dependents

  • Detailed, confidential description of the economic situation

  • Type of Music Industry Professional services conducted in the state of Hawaiߵi

  • Financial documents are not required; however, you will be asked to certify a detailed statement of this situation personally

Deadlines

Applications must be submitted by:

  • April 1 - June 30, 2023 for awards in July 2023

  • July 1 - September 30, 2023 for awards in October 2023

  • October 1 - December 31, 2023 for awards in January 2024

Completed applications must be submitted by 5:00 pm (HST) on the deadlines indicated above. The application automatically closes in the Grants Portal on these dates/times.

Processing of EME Grant Award

  • Applications will be evaluated and awarded from the 1st through the 14th of the month following the deadline

  • Award Letters will be emailed to the awardee with a request for a confirmation of receipt within a 7-day required return deadline

  • The EME grants will be distributed via check through the U.S. Postal Service Certified mail to those qualified within seven days of being notified that you have qualified

  • A receipt of the delivery signature will be required, and you will be responsible for any individual signing/receiving this mail at the address you provide in your application

  • A copy of the receipt of the purchase will be required to be emailed to HKF within 30 days of the award
